Dental Monitoring - Suitable for Children and Adults

Case report 1: A large anterior open bite due to thumb sucking

“I feel so amazing! So much more confident with my smile. I’m so thankful, definitely just thankful. I used to suck my thumb until I was 13 and I couldn’t bite lettuce for example, my mouth couldn’t close. I had massive trouble biting and I thought “I’m going to get this fixed.”

Problem:

At 13, Pearl had the classical signs of a long-established thumb sucking habit. She had an anterior open bite between her front teeth, which prevented her biting into fine foods, crowded teeth and a unilateral crossbite on the left side.

Solution:

After conquering her thumb sucking habit (which she says was the hardest part of treatment), she had orthodontic treatment with Invisalign clear aligners over 25 months, wearing elastic bands to pull her side teeth together into the correct position. Using the Dental Monitoring app on her phone, she scanned her teeth weekly, so that we could see her progress and avoid bringing her for in person appointments unless they were absolutely necessary, which meant her treatment was to her maximum convenience. She had a great result and only needed to attend the practice 12 times, without losing the close personal relationship between her and the orthodontic team.

Case report 2: Correction of prominent teeth

"I had a really massive overbite before and I really wanted to fix that because it was making me very uncomfortable to smile."

Rana’s upper teeth were prominent by nearly 8mm. Given a choice between conventional fixed braces or Invisalign, she chose to correct her bite with Invisalign. She still had to wear elastic bands, however! Fourteen months after starting treatment, Rana is delighted with the results – as are we.

Case report 3: A gappy smile

Problem:

This patient was very self-conscious of his gappy smile and didn't like to smile on photographs. Additionally, he had narrow upper lateral incisor teeth and large anterior gap (diastema).

Solution:

The spaces between his teeth were closed using Invisalign clear aligners over 13 months, with self-scanning using the Dental Monitoring app on his smart phone to monitor his progress, supplemented by minimal in practice visits. Once orthodontic treatment was completed, his narrow upper lateral incisors were increased in width using composite bonding by his dentist. As a consequence of treatment, his self-confidence has increased and he's happy to show off his lovely smile.

Case report 4: A misaligned class II bite and incisor crowding

Problem:

This young girl had a misalignment of her bite (a class II malocclusion) and mildly crowded teeth.

Solution:

She was treated using the Carriere Motion Appliance and Invisalign clear aligners. Alternative options for treatment include traditional fixed braces with elastic bands; extractions of upper premolars or headgear. Instead, we chose to use the most contemporary treatment options currently available by correcting the bite relationship swiftly with the Carriere Motion Appliance using elastic bands, then progressing to dental alignment and space closure with Invisalign clear aligners. Her treatment took 15 months, with 11 in house visits.

This case is a superb example of the excellent application of modern orthodontic techniques, which resulted in an exceptional outcome, conveniently and comfortably. The patient loved the comfort, aesthetics and convenience of this treatment modality. Her family is delighted with her great smile and the absolute ease of treatment.

Teenagers can commit to aligner therapy without parental involvement or losing their aligners!

Case report 5: A unilateral crossbite in an 8-year-old child

Problem:

This 8-year-old girl had a crossbite on the left side of her teeth, in that her upper baby (deciduous) teeth were biting on the wrong surface of her lower teeth.

Solution:

She wore a series of Invisalign clear aligners over a 6-month period and the crossbite corrected nicely. Invisalign is a simple alternative to a traditional orthodontics, such as a removable expander brace, to correct problems that appear in young children. Simple correction of this problem now will help the adult teeth erupt into the correct position and may mean avoiding further orthodontic treatment later.

Case report 6: An anterior open bite, unilateral crossbite, and incisor crowding

Problem:

This young man had a mild class III malocclusion, with a gap between his front teeth (anterior open bite) of 3mm, mild crowding of the upper incisors and a unilateral crossbite on the right side with minimal contact between his back teeth. He couldn’t bite properly and didn’t like his smile.

Solution:

Over 17 months, he wore Invisalign clear aligners, with elastic bands (class III traction) to correct his bite, with treatment progress monitored by self – scanning using the Dental Monitoring app on his smart phone, supported by occasional in practice visits. This treatment plan was very successful, delivered simply, conveniently and has remained stable over a 12 month follow up period. The patient is thrilled with the improvement in the function of his bite and the aesthetics of his smile.

Claire Nightingale comments on the technical aspects:

Managing anterior open bite is an unpredictable treatment that can both invasive and disappointing, with a high relapse potential. Invisalign clear aligners are a safe, effective, non - invasive method of successfully treating patients with an anterior open bite, provided mechanics are carefully considered, with intrusion of the posterior teeth being more reliable than extrusion of the anterior teeth.

Alternative treatment options include palatal temporary anchorage devices, with fixed appliances or orthognathic correction using a differential maxillary impaction. Both of these options are considerably more invasive and possibly less predictable than correcting the anterior open bite using Invisalign clear aligners with carefully considered differential intrusion mechanics to alter the occlusal plane.

I had been sceptical about using Invisalign clear aligners for the management of anterior open bites, but since the success of this case (and several others with a similar problem), Invisalign has now become my appliance of choice for managing this problem for patients whose skeletal contribution to the malocclusion is mild.

Case report 7: Missing teeth and a poor bite

Problem:

This lady had a mild class III malocclusion and multiple missing adult teeth that had simply failed to grow (hypodontia). She had some retained baby (deciduous) teeth. Her front teeth didn’t meet properly (anterior crossbite) which contributed to her adult teeth becoming progressively worn down as she ground her teeth at night (bruxism). She never had orthodontic treatment as a teenager, and her poor bite needed to be corrected before she had restorative dentistry.

Solution:

Over a 6-month period, her bite was corrected using Invisalign clear aligners and elastic bands (class III traction), with treatment progress monitored via the Dental Monitoring app and her smart phone, supplemented by occasional in practice visits. Once the orthodontic treatment was completed, she had an implant retained restoration placed to replace a missing lower left premolar and bonding to her deciduous teeth by a cosmetic dentist.

Case report 8: Correction of crowding and posterior crossbite

“I’ve been absolutely delighted. Claire was extremely patient and explained things to me. I was sometimes quite worried when I tried to put the next set of retainers in and I knew I had to do it and it was really quite an effort, but it’s been well worth it. I think you want to put a good face on the world and I think it’s a sad thing if people think just because they’ve stopped doing paid work on a daily basis that they should give up on their personal appearance. It’s almost more important as you’re getting older to maintain your image.”

Morfudd became very self – conscious of her smile after a boss suggested she should have something done about her teeth! On retirement, she decided the time had to come to invest in herself, (rather than the kids for once!) and create a smile to carry her through the next stage of her life.

In addition to having crowded teeth, she had a size mismatch in her jaws, which resulted in her upper left molars meeting on the tongue surface of her lower molars (a posterior crossbite). After only one year of treatment with Invisalign, Morfudd achieved a great result, improving both the look of her smile and the function of her bite. She had some restorative dental treatment with her own dentist to focus on the health of her teeth and even 10 years later, her smile looks fabulous.

Case report 9: Facial asymmetry causing an anterior crossbite

“I was born with one jaw which was slightly longer than the other which gave me a crossbite. I was very conscious of this throughout my entire life and just wanted to get it fixed. I’d been to see quite a few different dentists and orthodontists that gave me a variety of different options. You offered me a treatment involving no teeth being removed, which is what I really wanted. This was the most simple option and I thought it was the best way to treat the problem that I had. It’s been very straightforward. Overall, it’s been really simple and easy to manage. I came in two or three times to add things to my teeth, and I started to use elastics half way through. Otherwise I’ve managed it by myself and it’s been a great success.”

Problem:

Jenny had a crossbite due to asymmetrical facial growth and wanted to get this fixed.

Solution:

She had sought a range of opinions and chose to have the most simple treatment option, aligning her teeth without extracting any teeth or having jaw surgery to correct her asymmetry. She wore elastic bands (class III traction) to correct her bite, in addition to having minor filing between her teeth. Despite the Covid – 19 pandemic interrupting her treatment progress, she completed her treatment in 13 months and now has a much more comfortable bite, with a lovely smile and straight teeth that are easier to clean.

Case report 10: Crowded teeth and a reverse bite (anterior crossbite)

“Basically I’d had a very poor bite since childhood, which actually meant that the front teeth were clashing and it came to a head a few years ago when I had a holiday spoilt because I really couldn’t chew properly and it suddenly dawned on me: “Wait a minute! Why can’t dad have some orthodontistry, which both of my children have had with Claire? So I visited Claire in the hope that she would be able to do something with this bite that had been awful for 40-50 years and remarkably we were able to solve the problem. For a year I have been dutifully wearing the Invisalign; very comfortable without any issues of discomfort or pain; and combining proper 21st century orthodontistry with restorative work, I now have teeth which bite and chew very effectively but also are much more symmetrical and attractive than they were 50-60 years ago. So I wish we’d had this technology and I wish we had had Claire 50 years ago so it could have been done in my teens rather than in my 60s.”

Problem:

Howard’s upper jaw is small and narrow relative to his lower jaw. Consequently, he had a full crossbite of his upper teeth, which were also crowded. He found his bite very uncomfortable and ineffective. Also, in his early life, Howard’s previous dentists had made some unusual decisions which resulted in him having his upper right adult canine extracted and his right side lower incisors ground down in height to provide space for a crown on the upper right central incisor. He disliked the appearance of his teeth but was content with his facial appearance and after careful treatment discussions with both a maxillofacial surgeon and a restorative dentist, jointly with Claire Nightingale, he decided to have treatment without altering the size mismatch of his jaws.

Solution:

As space was needed to straighten his upper front teeth and then improve their cosmetics, we took the unusual decision to worsen his anterior crossbite by moving his lower incisors forward, out of the way of the upper teeth. He had his lower incisors repaired to their full anatomical height and a temporary crown of a better width placed on the upper central incisors before orthodontic treatment began.
A successful outcome was achieved very simply, by Howard wearing Invisalign clear aligners for a period of 17 months combined with cosmetic dentistry with Dr Tony Willetts to restore the height of his lower incisors, replace his crown and old dental restorations.

Case report 11: Correction of anterior crossbite

Jane had a long lower jaw which meant that her lower teeth trapped her upper teeth incorrectly (a crossbite). One of her upper incisors was worn down as a consequence. Jane had orthodontic treatment with Invisalign, including wearing elastic bands, for only 9 months. Once her bite was corrected, her dentist was able to restore her worn tooth beautifully with composite bonding.

Case report 12: Crowded lower incisors

Problem:

This lady’s lower incisors had become progressively crooked over recent years and she wanted to straighten them.

Solution:

She wore Invisalign clear aligners, with treatment progress monitored by Dental Monitoring, over 11 months, including two sets of additional aligners. Her teeth were held in position by fixed and removable retainers.

No one needs to know
you're wearing braces

We fully understand that, for many people, the one thing stopping them from undertaking orthodontic treatment is the thought of having visible braces on their teeth for anything up to two years or more. Crowded or protruding teeth may already be an area which causes a lack of confidence and, as such, it’s not uncommon to feel that braces will simply add to that feeling.
